Referring Veterinarians
To transfer a case to CVESC, please call 773 281-7110 and choose "option 1". An ER veterinarian will be able to coordinate your transfer. You may also fax the downloadable transfer form with any relevant records, diagnostic tests, and requests. If you want to refer to a specialists, please download the referral form and fill out as completely as possible.
